  We are looking for a Registered Nurse to join our Team in Albuquerque, NM.

  This full-time job is located at New Mexico Heart Institute, the largest private cardiology practice in the state serving patients in 25 different New Mexican communities, with care delivered in Albuquerque and Santa Fe. NMHI practices state-of-the-art medicine by employing the latest cardiovascular imaging and fully integrated interventional, cardio-thoracic, and vascular surgical approaches. The Institute’s services, a number of which are conducted in conjunction with the Heart Hospital of New Mexico, located next door, address a full-range of preventive, diagnostic, treatment, surgical, and rehabilitation needs.
  Registered Nurse Job Responsibilities:

  • Collaborate with the clinical staff, physicians and other members of the patient’s healthcare team, to develop an individualized and comprehensive plan of care according to the applicable program specifications and guidelines
  • Oversees the coordination and facilitation of care transitions
  • Utilize clinical judgment, independent analysis, critical thinking skills, time management skills and detailed knowledge of case management program and complies with quality standards
  • Coordinate with hospitals, internal and external providers to ensure the coordination and integration of services as planned in the case management program, while revising the plan as needed to provide high quality continuity of care services and cost-effective service delivery

  • Acts as a liaison for assigned patients and advocate for resources and removal of barriers

  • Provide education to patients and/or caretakers regarding procedures, medications, lab test results, etc.

  • Facilitate and promote full cycle health maintenance, wellness programs, and heart disease management for patients

  • Performs care management services within Medicare and other insurance guidelines to maintain compliance with care management regulations
  • Collaborate with post-acute care partners to identify high risk of readmission patients and tailor post-acute care planning
  • Follow patient safety-related policies, procedures, and protocols and report potential or actual patient safety concerns and medical errors in a timely manner

  • Other duties as assigned

  • May be required to provide emergent coverage at Heart Hospital when needed during work hours.

Registered Nurse Education and Experience Requirements:

  • High school diploma or GED required

  • Graduate of NLNAC accredited school required

  • New Mexico RN or LPN licensure required

  • BLS and ACLS certification required
