Community Health Worker, Labor of Love, PRN job,Variable Hours  Albuquerque, NM

  We are looking for a Community Health Worker to join our Labor of Love team in Albuquerque, NM.

  This PRN job is located at Lovelace Women’s Hospital, the first and only hospital in New Mexico dedicated to women’s health, offers a 53-bed advanced neonatal intensive care unit, a natural birthing center, an award-winning breast care center and the first robotic surgery program in the state recognized as a Center of Excellence in Robotic Surgery by Surgical Review Corporation.

  Community Health Worker Job Responsibilities:

  •   Lovelace Labor of Love is our maternity navigation team, supporting pregnant women and their families regardless of birth plan or insurer.
     
  •   Coordinate and provide services and resources to an individual/family necessary to develop and facilitate a personalized intervention plan based upon barriers and health care issues identified through telephonic and/or on-site screening process .
  • Facilitates appropriate utilization of resources through identification of alternatives, prevention of duplication/inappropriate use of services through on-going communication with patient/family and payer source.
  •   Intervention plan and case related documentation is completed on all patients within the time frame of the established policy. 

Community Health Worker Education and Experiences Requirements:

  • High School Diploma or GED Equivalent, Associate’s Degree or Degree from Accredited College desired
  • One (1) to Three (3) years recent related experience
  • Certified Community Health Worker preferred
     
  • Bilingual in English/Spanish preferred
  • Knowledge of Medical Terminology
  • Able to work variable shifts- day, evening, some weekends
     
  • Ability to communicate effectively, function independently, manages time efficiently, organize work flow and maintain confidentiality
  • Ability to balance business’ needs with patient advocate services
  • Knowledge of community and state resources
  • Strong analytical and problem solving skills
  • Computer skills
