Anticoagulation Nurse, RN, NMHI, Part-Time, Days 

We are looking for a Anticoagualation Nurse, RN to join our team in Albuquerque, NM 

 
This full-time job is located at New Mexico Heart Institute, the largest private cardiology practice in the state serving patients in 25 different New Mexican communities, with care delivered in Albuquerque and Santa Fe. NMHI practices state-of-the-art medicine by employing the latest cardiovascular imaging and fully integrated interventional, cardio-thoracic, and vascular surgical approaches. The Institute’s services, a number of which are conducted in conjunction with the Heart Hospital of New Mexico, located next door, address a full-range of preventive, diagnostic, treatment, surgical, and rehabilitation needs.
   
Anticoagulation Nurse, RN Job Responsibilities:
 

  • Perform finger sticks (PT/INRs); assess patients for anticoagulation status. 
  • Provide patients with education and instruction regarding dosing, medication side effects, and safety precautions for anticoagulation therapy.
  • Triage patient issues ranging from minor complaints to emergency issues. 
  • Identify patients who fall outside the protocol guidelines and bring such patients to the attention of the cardiologist.
  • Identify patients whom home monitoring is appropriate for; provide telemanagement for patients who are unable to come into the clinic.
  • Ensure complete and accurate documentation of nursing activities and information using an EMR system. 
  • Obtain and review patient lab results; call patients in a timely manner with laboratory results and plan for follow up including any medication changes, future laboratory needs, and/or scheduling office visits. 
  • Grant refill approval or denial for medications request from pharmacy line, faxed or written prescriptions based on established protocols. 
  • Coordinate patient care with hospitals, home health and outside physicians’ offices.

  Anticoagulation Nurse, RN Job Education and Experience job Requirements:

  • High school diploma or GED required
  • Graduate of NLNAC accredited school required
  • New Mexico RN licensure required
  • CPR and ACLS certification required
  • Minimum two (2) years nursing experience in anticoagulation, case management, and/or cardiology preferred.
  • Ability to perform in potentially stressful, quickly changing, working conditions.
  • Demonstrated experience working with high level professionals requiring outstanding expertise and professionalism.
  • Experience working with diverse populations with limited understanding of healthcare processes and procedures; must be able to provide complicated information in a clear and concise manner to those not familiar with the information being provided.
  • Problem solving skills such as researching information; the ability to determine if information found is valid and the ability to make decisions with information gathered.
  • Bilingual in Spanish or local Native languages (oral and comprehension) a plus.
  • Must possess strong organizational, time management, detail and communication skills; the ability to prioritize projects and work in a fast paced, frequently crisis driven environment.
  • Computer skills required, including Microsoft Office Suite, e-mail, and typing.
